We do much the same, except that we're usually the ones starting a Cayuse record, so if there's a need to create a new sponsor profile, we just do it. I don't think we even created a form for that. We have Banner access ourselves, so we can check to see if the sponsor is already in Banner and, if not, create that profile and generate the sponsor number to enter into the Cayuse profile.
As to what we enter, typically it's at least the business address, website, sponsor type, and DUNS number. (Though, note to Cayuse, will that field be changing to UEI at some point?)

Requests for new sponsors or subs are emailed to my team who sets them up into Cayuse.
We review the current sponsor/sub list to see if the entity is already in the system to avoid any duplicates.
If not there, we contact our research accounting office for a sponsor number in our financial system (Banner).
We enter that number in Cayuse for easier reporting, especially on expenditures. Hope this helps.
