The EPA 4700-4 Version 5.0 form was released by Cayuse on February 19, 2022. It has been confirmed that the form underwent further modifications without public notice, resulting in errors encountered by Proposals (S2S) customers applying for a recent EPA funding opportunity.

The corrected form will be available Saturday, April 16, 2022, at 3:00 PM EST (12:00 PM PST). Customers will be able to do their proposal at that time.

Cayuse recognizes the importance of having up-to-date forms and validations to ensure error-free submissions for our customers. Going forward, we will verify if any additional form updates are made two weeks prior to the agency's production release date in order to mitigate future submission issues.

    Ed,

We were victim to this.  Can you confirm that the EPA has give us permission to submit a new application after the deadline?

    Ted:

You will need to contact EPA for a waiver. I saw that a couple of funding opportunities closed on 4/11:

    EPA-I-OCSPP-OPPT-FY2022-001
    EPA-HQ-OPPT-2022-001

    The POC for both is:

    Michele Amhaz
    amhaz.michele@epa.gov
    Phone: 202-564-8857

Unfortunately, we don't know the fine details of when the changes occurred. Our team was proactive in developing the form Jan/Feb once the specs were posted on Grants.gov. It appears that the funding opportunities were originally posted on Feb 8, and then modified by EPA on Feb 18. So at that time, the forms were "in sync".

I communicated to Grants.gov (since they do the forms development on behalf of all of the federal agencies) that it would be extremely helpful to proactively send a communication for any form changes (even if it's at the 11th hour). That way we can work with our customers and ensure submissions.

It may be helpful for Grants.gov to hear that message as well from institutions. The has been a recurring issue.
